Outcome monitoring tracks the progress of an intervention using outcome measures, usually before and after (or pre and post) the intervention. Outcome monitoring may not be possible for some types of interventions-- e.g. street outreach or media campaigns where it is difficult or impossible to collect baseline and follow-up data.
Outcome evaluation is the over-all evaluation that assesses whether the intervention is achieving the desired effect(s). The objectives should be set ahead of time. And the objectives should link theory and available research to the results being targeted in the program.
Outcome indicators are the specific things that are measured. Indicators are the specifics of the goals and objectives; they are clearly defined and can be measured. Outcome indicators often focus on the shorter-term changes that occur in participants as a result of the intervention.
